YOU ARE NOTIFIED that an action for Declaratory Judgment has been filed against you to have the Peak Property and Casualty Insurance Corporation policy of insurance #11409189115 that was issued to Gregory Hill with an effective date of 02/01/2024 declared null and void for misrepresentations, omissions, concealment and incorrect statements that were material to the acceptance of the risk and the hazards assumed. You are further notified that you are required to serve a copy of your written defenses, if any, to Robert E. O’Connell, Esq., Plaintiff’s attorney, whose address is 210 N. University Dr., 6th Floor, Coral Springs, Florida 33071, tel: (954) 340-0005, on or before 01/09/2026 and file the original with the Clerk of the Court.

If you fail to do so, a default will be entered against you for the relief demanded in the Complaint.
