TO: CMH FITNESS, LLC, doing business as RUKUS CYCLING TAMPA, MELANIE HENRY, and CONRAD HENRY, the above-named defendants.

You are hereby required to appear and defend the complaint filed against you in the above entitled action within thirty (30) days from the date of the first publication of this summons, and in case of your failure to appear and answer, plaintiff for want thereof will apply to the above entitled court for the relief prayed for in its complaint, to-wit:

A. FIRST, SECOND, AND THIRD CLAIMS FOR RELIEF, JOINTLY AND SEVERALLY:

1. For the principal sum of $127,311.77, plus accrued interest at the default rate of 18.00 percent per annum from March 12, 2025, through May 9, 2025, in the amount of $3,641.46, plus default interest accruing on the principal balance at the rate of 18.00 percent per annum ($62.78 per diem) from May 10, 2025, until the date judgment is entered;

2. For Plaintiff’s reasonable attorney fees, costs and disbursements incurred herein;

3. For interest on the sum of paragraphs A1 and A2 at the contract default rate of 18.00 percent per annum, from the date judgment is entered until fully paid; and

4. For such other relief as the Court deems just and proper.
